CreekStone Academy agrees to provide child care for the aforementioned children during the hours specified. My child(ren) will participate in the breakfast, lunch, and afternoon snack meal plans when present.
Before any medication is dispensed to my child(ren), I will provide a written authorization, which includes: date; name of child; name of medication; prescription number, if any; dosage; date and time of day medication is to be given. Medicine will be in the original container with my child's full name marked on it.
My child(ren) will not be allowed to enter or leave the facility without being escorted by the parent(s) or person authorized by the parent(s), or facility personnel.
I acknowledge that it is my responsibility to keep my child(ren)'s records current to reflect any significant changes as they occur, i.e. telephone numbers, work location, emergency contacts, child's physician, child's health status, infant feeding plans and immunization records, etc.
The facility agrees to keep me informed of any incidents, including illnesses, injuries, adverse reactions to medications, etc., which include my child’s adverse reaction to medications, etc, which include my child.
CreekStone Academy agrees to obtain written authorization from me before my child participates in routine transportation, field trips, special activities away from the facility, and water-related activities occurring in water that is more than two (2) feet deep.
I agree to pay the total weekly fee specified by the school.
I agree to provide the center with all necessary information pertaining to administering medicine to my child(ren).
If my child wears diapers, l understand I will provide whatever disposable diapers are necessary for my child.
Should my child become ill during the time that he or she is in the care of CreekStone Academy or suffer an accident of any nature, the academy shall contact me immediately and shall be authorized to secure such medical attention and care as may be necessary. (The parent shall assume responsibility for payment). I agree to keep the center informed as to changes in telephone numbers, etc. where I may be reached.
I understand that if my child is ill, including, but not limited to, a severe cough or sore throat; undetermined rash or spots,temperature over 100 degrees; severe headaches, upset stomach or diarrhea, he or she cannot be accepted or return to the academy until well. In the event my child has a communicable disease, a release form from a medical source may be required before my child re-enters the academy. CreekStone Academy will notify parents if a communicable disease has been introduced to the academy.
I have received a copy and agree to abide by the policies and procedures for the CreekStone Academy.
